Go to
and open a new blank form.forms.google.com Click the three dots (More) in the top right corner.
Select "<> Script editor."
Step 3: Paste and Run

Delete any text in the Script Editor and paste the code from the Gem.
Click the Save (Disk) icon and then the Run (Play) button.
